SAMPLE MESSAGE of accommodation notification to instructors:

    Dear Instructor (___NAME__),

I am a student registered in your online course (COURSE SUBJECT, NUMBER and SECTION) for Fall 2014.

I am a student with a documented disability and have registered for accommodations through the WSU Access Center.

Attached is the accommodation form that outlines the reasonable accommodations for my disability.  Please take a moment to read this form.  If you have any questions about my accommodations, please let me know.  I am happy to discuss my accommodations with you.  I also ask that you read the Flexible Attendance Guidelines.

Please note that this form is for your information—the Access Center is not requiring that you sign it and send it back.

If there are questions I can’t answer, my WSU Access Advisor (Meredyth Goodwin; m.goodwin@wsu.edu) will be happy to talk with you.

Also, if my accommodations state I require additional time (e.g.; double time) to take online exams or quizzes, then it is your responsibility to set up my time extension for every online timed event (e.g.; online exams, quizzes or assignments) in your course. Setting these extensions should be completed at least a week before any timed event is to be completed.

If you need assistance regarding online testing accommodations or would like someone to check that your online exams and quizzes are set up correctly, please contact  WSU Global Campus Online Faculty Support at wsuonline.support@wsu.edu  or 509-335-3557

    Thank you for your support.

    Sincerely,

    YOUR NAME and ID#
